We will file insurance and collect only your deductible and copayment for the managed
care plans we participate with, which are listed below. If your insurance company is not
listed, please look under the name of the managed care network it uses (for example,
Beech Street, MedCorp Southwest, or PHCS). If you have other insurance we will collect full payment at the
time of service and give you a form to file with your insurance company for reimbursement.
Payment for medical services is ultimately the patient's responsibility regardless of
insurance coverage.
Some plans require preauthorization for DXA. Please check with your insurance company
about this. The CPT code is 76075.
